This is a combined symposium dealing with the transition of care in pediatric hematology and oncology, and adolescents and young adults (AYA) diagnosed with cancer. The first part of this symposium will address transition of care in pediatric hematology/oncology. Health problems experienced by adult survivors of childhood cancer will be addressed. The various issues that need to be considered in building a transitional care program for pediatric oncology will be discussed. Later, the multidisciplinary needs and psychosocial issues faced by the adult sickle cell disease population will be addressed. The second part of this symposium will address the various issues faced by the AYA relating to diagnosis, treatment and participation in clinical trials, and approaches to problems encountered.
